REFERRALS
If your insurance company requires a referral to see a specialist, you need to obtain the referral and fax it to your Surgical Clinic surgeon's office before the appointment. Your appointment will have to be rescheduled, if the office hasn't received the referral before your scheduled appointment. Find fax numbers on our "Locations" page.
MEDICAL RECORDS RELEASE AND CONFIDENTIALITY
To maintain strict confidentiality* of our patients, we require written permission* from the patient before any records can be released to another physician, insurance company or any other person.
**Fill out our medical release form and fax or mail it to the appropriate location. Once this form is received, copies will be sent ccordingly.

INSURANCE AND DISABILITY FORMS
Please bring any forms that need to be filled out to our office. There is a $10 charge for "out of work/disability" forms to be completed, which will be collected when the forms are presented to our office.
IN CASE OF EMERGENCY
A physician is always available by phone in an emergency 24 hours a day by calling the office numbers listed. If your call is after hours, our answering service will contact your physician or the physician on call. If you or your family feels the situation is life-threatening or demands immediate attention, go directly to the emergency room nearest you.
