Once this form is received, copies will be sent ccordingly. *Please read and be familiar with our Privacy Statement. INSURANCE AND DISABILITY FORMS Please bring any forms that need to be filled out to our office. There is a $10 charge for "out of work/disability" forms to be completed, which will be collected when the forms are presented to our office. IN CASE OF EMERGENCY A physician is always available by phone in an emergency 24 hours a day by calling the office numbers listed. If your call is after hours, our answering service will contact your physician or the physician on call.
